That's what makes me think it refers to the child's objective position in society (or more to the point, their age) rather than their relative position in the family. Ena's is 中 (nakawaka?) but she's the youngest of her family, so she should be of the lowest status, yet she ranks higher than Yotsuba who is an only child.
Although I suppose a child's status within their family could be arbitrarily determined by age, rather than by their relative position of succession of the family line.

If the board rules don't allow animated depictions of Fuuka's ass, they certainly won't allow photographic depictions of a child's ass.
